- With the submission of this form the Mission Trip Group acknowledges they have read and will adhere to the Highlights for Mission Team Participation document. The Mission Center is not reserved for anybody until this Group and Information Application Form is received.
- Reservations are not confirmed until the Registration Fee/Deposit from the Group is received and will be cancelled if the required fees from the Group is not received in full.
- The Registration Fee/Deposit is due no later than 60 days prior to the trip date.
To be able to fill out the Group Application and Information Form (pdf), you need to FIRST CHECK the Hands of Hope Calendar to see the dates of other teams that have already signed up and have been approved. Yet it is always a good idea to check with Crystal Green, executive director, if a group has specific days in mind for a trip. Sometimes a mission team will postpone or cancel a trip unexpectedly and dates will open up; for example, some trips were postponed due to the COVID-19 pandemic. The form looks like this below:

And also remember to send the Group Registration Fees/Deposit AND individual participant HOUSING fees by required date timelines:
(1) A group registration fee (or deposit) is $40.00 per person is due no later than 60 days prior to arrival date. In This fee reserves the project date for your group and will be applied to the total balance due. A confirmation form will be mailed/emailed to you upon receipt of deposit. This also reserves the housing dates at the Mission Center.
(2) A fee of $25 per night per person for the cost of staying at the Mission Center is due no later than 2 weeks prior to the arrival date.
(3) Any remaining balance is due no later than 2 weeks prior to arrival date.
(4) Any Project Fees are due upon arrival.
