Dear Mission Ministry Partner or Leader,
Thank you for your interest in bringing a mission team to southwest West Virginia. Hands of Hope Ministries, Inc. is dedicated to “reaching out to others” by bringing a message of hope through the love of Jesus Christ. Trip information forms are available on this website (hohm.org) or can be mailed to you upon request. Please review the following guidelines and give us a call if you have any questions.

Group Application: MAIL OR EMAIL ONE GROUP FORM TO RESERVE HOUSING. No dates are reserved until this is received.

Registration Fee/Deposit: MAIL/PAY ONLINE REGISTRATION FEE/DEPOSIT OF $40 PER PERSON. The registration fee/deposit is due no later than 60 days prior to arrival date. Reservations will be voided and open to other groups if registration fee/deposit is not received on time. Remaining balance is due two (2) weeks prior to arrival.

Participant Application: ONE FORM MUST BE SUBMITED FOR EACH PARTICIPANT. Please make copies. Each person must fill out, sign and return this application form – NO EXCEPTIONS. It must be received two weeks prior to arrival date. No one is allowed to stay without this form. (Note: A new form is required for each stay at Hands of Hope’s Mission Center)

Medical Release Form: DO NOT MAIL THIS FORM TO HANDS OF HOPE. Please make copies. Make copies for each participant and mission trip leader. Copies must be NOTARIZED and a copy of the front & back of insurance card must be attached with the Medical Release Form. Mission Trip Leader must collect all Medical Release Forms prior to departure to Hands of Hope facilities and MUST keep with them AT ALL TIMES.

Rules and Regulations: Please make copies for each participant. All mission trip group members MUST READ and SIGN the agreement for the Rules and Regulations on the Participant Application Form.

Packing Lists/Directions: Please share with group and send them to the Packing and Directions page of this website.
As you prepare for your trip, we recommend that you have meetings to organize details and discuss the focus of your mission. Join us in prayer that service that you will perform will not only transform the lives of those we serve but also the lives who come to serve.
